[edit] Purpose
House Resolution 2 requires that when any appropriations bill contains funding for an entity that is not a budget unit or a political subdivision of the state, it must be submitted with additional information. If the bill supplemental information form is not received, filed, and made a public record, and made available to all members of the house, then the bill will not be read or voted on.
[edit] Supplemental Information Form
The supplemental information form will include the following information:
- The recipient's full legal name and mailing address
- The type of entity: and if it is a corporation, the name of the incorporators
- The last four numbers of the taxpayer's identification number of the receiver
- The name of the legislator who is the requestor
- The name of each member of the recipient entity's governing board and officers
- The dollar amount to be appropriated.
- The recipient's proposed budget for the funds.
- The recipient's public purpose sought to be achieved through the use of these monies. This must be summarized in a detailed manner so that any reasonable citizen would understand the true nature of the purpose.
- The proposed length of time the recipient estimates it will take for completion.
[edit] State Official Involvement
If any elected or appointed state official or an immediate family member is an officer, director, trustee, or employee of the recipient, the following needs to be documented in the supplemental information form:
- The name and address of the official and the office held.
- If an immediate family member is involved, the name and address of that person, the position held, and thier relationship to the official.
- The percentage of the official's, or immediate family members ownership interest in the recipient entity.

The Clerk of the House of Representatives shall create and provide the supplemental information form required. The information will be published by the Clerk and will become a public record available via the internet. [1]
